Roots is based in Simpsonville, South Carolina & offers herbal consults by phone throughout the US.

Please print and fill out a new patient intake form before your first visit. Do not send these forms by email.

CANCELLATION AND NO-SHOW POLICY:

If you need to cancel or reschedule your appointment, we require a minimum 24 hours notice. Any cancellations made with less than 24 hours notice are subject to a charge of 50% of the scheduled service fee. A no-show (missed appointment without given notice) will be subject to a charge of 100% of the service fee.
